Sheri is the Homecare Operations Manager at Breakthrough Case Management, in addition to this pivotal role within the company she maintains a specialised case load as an associate case manager.

 

Based in Wiltshire, Sheri is a highly experienced, confident and capable senior clinical manager with an outstanding track record in the care sector including CQC registered positions.

 

Starting her career as a theatre nurse working in Orthopaedic and Trauma Theatre, Sheri then worked as a general nurse in the older care sector while bringing up her family. 

 

Twenty-one years ago, Sheri found her passion; working with and supporting individuals who live with complex physical disabilities, neurological disease, neurological disability and learning disabilities.  Sheri’s career history includes the management of complex care homes; supporting individuals living with life limiting conditions; managing staff teams and working within guidelines and regulations to provide excellent care.  Sheri is an experienced clinical manager who is skilled in supervision, mentoring, training, coaching and teaching.

Sheri has broad experience working with complex case management clients, planning rehabilitation, setting therapeutic goals and co-ordinating the activity of clinical teams and ongoing referrals. Sheri regularly works in collaboration with multiple clinical disciplines including, but not limited to neurologists; medical and surgical professionals; physiotherapists; occupational therapists; speech and language therapists.  Sheri has additional strategic experience working in partnership with Local Authorities, CCG’s and NHS services to co-ordinate, initiate and deliver streamlined, seamless, safe and effective care and rehabilitation solutions for her clients.
