Our experience shows that successful rehabilitation occurs when there is a strong relationship between you, your carers and your therapy team. Our carers and rehabilitation assistants work on a 1-to-1 or 2-to-1 basis, giving you the opportunity to build a solid relationship with your team and play an active role in your own rehabilitation.
We recruit, train and manage a team to meet your unique needs. We will work with you to find the best way forward and set goals that are achievable and pertinent to you. With Breakthrough, rehabilitation never stops.

Breakthrough Care is an expert provider of 24 hour complex care packages. Breakthrough carers receive additional bespoke training from our partner neurophysiotherapists, depending on the individual needs of each client.
Breakthrough Care delivery is enhanced and supported through the use of NHSX approved digital care planning and monitoring software. Giving care commissioners statistical data to support and evidence outcomes.
